Panchakarma Therapy in Ayurveda: How the Treatment Works

Panchakarma therapy in Ayurveda follows a structured treatment process. The therapies used can vary depending on the person's health condition, constitution and the physician's assessment.

1. Purvakarma (Preparatory phase)

1. Purvakarma (Preparatory phase):

The treatment may begin with preparatory therapies such as Snehana (including Abhyanga, or Ayurvedic oil massage), and Swedana, or therapeutic sweating. These therapies are traditionally used to prepare the body before the main procedures.

2. Pradhankarma (Main procedures)

2. Pradhankarma (Main procedures):

The main stage involves selected Panchakarma procedures. These traditionally include Vamana, Virechana, Basti, Nasya and Raktamokshana. Not every patient needs all five procedures.

3. Paschatkarma (Post-treatment phase)

3. Paschatkarma (Post-treatment phase):

After the main treatment, your physician may recommend dietary and lifestyle changes to help you gradually return to your normal routine. The aftercare plan depends on the therapies received and your individual requirements.

At Healing Earth Ayurveda Hospital, the treatment plan is monitored by our team of doctors throughout the program.

The 5 Steps of Panchakarma: Procedures We Offer

Panchakarma has five classical cleansing procedures. After examining your pulse, digestion and health history, your physician decides which ones you need, in what order and for how long. Most patients receive one or two main procedures, not all five. Every program begins with preparatory oil massage and steam therapy.

Vamana (Therapeutic Emesis):

What it is: A supervised cleansing for excess Kapha. After several days of internal and external oil therapy, you take a medicated preparation that induces controlled emesis, followed by a light diet to restore digestion. Traditionally used for: Chronic cough, bronchial asthma, sinus and allergic rhinitis, skin conditions such as psoriasis and urticaria, and weight concerns. Program: 7–8 days. Not suitable during pregnancy, acute illness, or with significant heart disease or uncontrolled blood pressure.

Virechana (Therapeutic Purgation):

What it is: A medicated purgation therapy for Pitta imbalance. You first take medicated ghee for several days, then a herbal purgative on the main day, with a gradual diet afterwards. Traditionally used for: Hyperacidity and gastritis, chronic skin conditions such as eczema and psoriasis, acne, migraine and constipation. Program: 7–8 days, or a single-day Sadya Virechana for selected cases. Not suitable during pregnancy, acute diarrhoea or severe weakness.

Basti (Medicated Enema Therapy):

What it is: Medicated oil or herbal decoction given through the rectal route. It is considered the main therapy for Vata disorders. Your physician chooses the type: Matra Vasti (oil), Kashaya Vasti (decoction), Ksheera Vasti (milk-based), or the 8-day Yoga Vasti, 14-day Kala Vasti and 21-day Karma Vasti courses. Traditionally used for: Arthritis, sciatica, lumbar spondylosis, chronic back and joint pain, constipation, IBS and certain neurological weakness. Program: from a single sitting up to 21 days. Not suitable during pregnancy, acute diarrhoea or active rectal bleeding.

Nasya (Nasal Therapy):

What it is: After a gentle face and head massage with steam, medicated oil or herbal drops are applied through the nostrils, followed by gargling and herbal inhalation. Traditionally used for: Chronic sinusitis, allergic rhinitis, migraine and headaches, neck stiffness and cervical spondylosis, stress and sleep concerns, and hair fall. Program: usually 7–14 days. Read our detailed guide: Nasya Therapy in Ayurveda: Benefits, Procedure and Cost in Bangalore.

Raktamokshana (Bloodletting Therapy):

What it is: A localized blood-purification procedure done by a trained physician, using leeches (Jalouka), vein puncture (Siravedha) or small superficial incisions (Prachana), depending on the condition. Traditionally used for: Localized skin conditions, non-healing ulcers, gout-related swelling, and persistent localized pain or inflammation. Program: usually a short course of sittings. Not suitable for anaemia, bleeding disorders, people on blood thinners, or during pregnancy.

Specialized & Para-Surgical Therapies:

Along with Panchakarma, our physicians may recommend specialized Ayurvedic procedures such as Uttara Vasti, Kshara Karma, Kshara Sutra, Agnikarma and Siravyadha for selected conditions.

Preparation Before Your Panchakarma Treatment

Preparing properly can make the treatment period easier to manage. Your physician may recommend the following before starting Panchakarma:

Initial consultation & diagnosis:

Your physician will discuss your health history, current concerns, lifestyle, digestion and other relevant factors before preparing your treatment plan.

Dietary adjustments:

You may be advised to follow a lighter and easily digestible diet before certain procedures. The recommendations will depend on your treatment plan.

Lifestyle guidance:

Patients may be advised to reduce strenuous physical activity and maintain adequate rest before and during treatment.

Medical review:

Inform your physician about existing health conditions, medications, allergies and previous treatments. This helps the medical team determine which therapies are appropriate.

Mental readiness:

Panchakarma requires time and attention. Patients are encouraged to keep their treatment period relatively free from demanding work schedules and unnecessary travel.

Duration: 7-Day, 14-Day & 21-Day Panchakarma Programs

Panchakarma does not follow one fixed duration for every patient. At Healing Earth Ayurveda Hospital, the treatment period depends on your health condition and the therapies recommended by your physician.

7-Day Program:

A shorter program may be suitable for selected patients seeking general wellness support or a less intensive treatment schedule.

14-Day Program:

A 14-day program provides more time for preparation, selected therapies and recovery. It may be recommended when the physician considers a longer treatment period appropriate.

21-Day Program:

A 21-day program provides additional time for the different stages of treatment and recovery. It may be considered for patients who require a longer and more structured treatment plan.

Aftercare Following Your Panchakarma Program

The treatment period does not necessarily end when the main therapies are completed. Your physician may provide an aftercare plan based on the treatment you received.

Gradually returning to your regular diet
Dietary recommendations
Herbal medicines where appropriate
Daily routine guidance
Advice regarding physical activity
Adequate rest
Follow-up consultations
