In this procedure, lukewarm medicated oil is retained over the knee joint for a specific duration using a leak-proof dough ring made of black gram flour. It is carried out by two trained therapists under the supervision of an Ayurvedic physician.

Janu Vasti works through transdermal absorption, thermal stimulation, and pharmacological action:
- Medicated oils penetrate the skin due to their lipid-based nature, which matches the phospholipid structure of cell membranes.
- Localised temperature rise of 2–3°C leads to vasodilation, improving blood supply and aiding in the removal of toxins and free radicals.
- The oils have anti-inflammatory, analgesic, and muscle-relaxant properties, providing pain relief and reducing joint inflammation.
- Acts as a counter-irritant, helping to modulate pain signals and reduce sensitivity.
- Improves muscle contraction efficiency through enhanced oxygen and nutrient delivery.
